Does a mobile prefix like 076, 078 or 079 tell me which operator a Swiss number belongs to?

No. Since portability arrived in 2000, a 076 number first allocated to Sunrise may now be served by Swisscom or Salt. Standard shows the original allocation; use mode=current for the operator actually serving the number.

Do I keep the leading 0 when formatting a Swiss number in E.164?

No. The national trunk prefix 0 is dropped internationally, so 044 202 xx xx becomes +41 44 202 xx xx and mobile 079 xxx xx xx becomes +41 79 xxx xx xx.

Why do some Swiss numbers have a 3-digit area code and others a 4-digit one?

The plan mixes a 3-digit code with a 7-digit subscriber number and longer codes with shorter numbers; either way the national significant number is always nine digits.
